Unlike previous volumes, this time there are virtually no clues if the school featured in the story is actually the same school from earlier works of Dr.P or a completely new one. Another drastic innovation is the fact that in addition to familiar school romance plots we now have ninjas to participate in and spice up the story, starting with titular Mikage-san, who is involved in a relationship herself plus is also always happy to help other couples in trouble by providing various useful ninja items and advices, continuing with an easy-going tomboy ninja head over heels into her ex-teacher and ending with a male ninja loyal yet somewhat jealous of the Ojousama he serves to.
